Instructions:

1. Save a form in PDF format to your desktop
2. Open with Adobe Reader or Adobe Acrobat
3. Complete all required fields
4. Save it
5. Close the PDF, then re-open it to ensure the content you filled in has been saved
6. Submit it to Accessibility Services via email
