DISABILITY Tax Credit Forms
Please be advised, the BCIT physicians will NOT be able to fill out Disability Tax Credit forms as it is not appropriate for a post-secondary institution clinic to be filling out these forms for students.
To qualify, you need to have a “severe and prolonged impairment that restricts your ability to perform mental functions necessary for everyday life”. Please visit the Disability Tax Credit website for more information.
We apologize for any inconvenience and appreciate your understanding.
Functional Abilities Forms
Please be advised that BCIT physicians are not able to complete Functional Abilities Forms.
This form is typically used by employers to determine how to accommodate an ill or injured employee so they may remain at work, or, if an absence is unavoidable, return to work as soon as it is medically safe to do so. In some cases, employers may also request this form prior to the start of employment.
Unfortunately, our clinic does not have the required equipment or appropriate space to perform the functional testing necessary to accurately complete this form.
We apologize for any inconvenience and appreciate your understanding.
