Medical documentation obtained within the last three years is required to request services and/or accommodations through the DRC. We may advise that some aspects of the documentation be updated. Accommodations and services are based on the current impact of the limitation or barrier in a post-secondary setting.
Documentation signed by medical doctors, registered psychologists, and other health professionals who have specific training, expertise, and experience regarding the disability must be provided.
Documentation must include:
- current functional limitation
- impact on studies and daily activities
- current treatment
- duration, severity, prognosis
- medication and side effects
- requests for adaptive equipment.
Please note that DRC does not assess students for a disability.
Further information can be found in Documentation Specifics.
