Supporting documentation is needed in order for the Disability Resource Centre to recommend accommodations.
For students already approved for DRC exam accommodations, download the Exam Scheduling Request Form E-mail, fax, or bring to the DRC.
In order to receive accommodation in the DRC, your Individual Accommodation Plan (IAP) has to be updated every term. Please contact us at your earliest convenience to set up an appointment for this purpose.
If you prefer, you can also complete the forms on paper by coming to the DRC office in person.
